Records Requests

The Records Requests menu provides quick access to tools for generating VA‑ready request letters and forms. These tools help Veterans obtain the records needed to support their claims.

Request C&P Exam (Privacy Act)

Generates a Privacy Act request letter for obtaining Compensation & Pension exam results.

Request C‑File (FOIA / Privacy Act)

Creates a FOIA/Privacy Act request for the Veteran’s complete claims file (C‑File), including rating decisions, evidence, and correspondence.

Request VA Specialty Clinic Records

Produces a request letter for records from VA specialty clinics such as cardiology, neurology, orthopedics, and mental health.

Request Community Care Records

Generates a request for records from non‑VA providers who treated the Veteran under the Community Care program.

Request Service Treatment Records

Creates a request for STRs, including medical, dental, and deployment‑related treatment documents.

Request DD214 and Corrections

Provides a request letter for obtaining a DD214 or submitting corrections through the appropriate channels.

Release Authorization (VA Forms 21‑4142/a)

Generates the authorization forms needed for VA to obtain private medical records on behalf of the Veteran.
